How to collect the history from the patients?

History collection is the important procedure in the nursing process.It is the starting procedure of nursing process.

What is history collection?

To collect the different types of datas from the patients to the way of  through the good communication with the patients for further treatment. It is the basic point of nursing process and nursing care.

History collection components

Bio Graphical data:

Name
Age
Sex
Marital status
IP . No
Ward
Address
Language spoken
Education
Occupation
Date of admission
Cheif complaints
Diagnosis

History of present illness:

Patient's present complaints ,disease condition and diagnosis .

History of past illness: 

The past medical/ surgical history of patient should be collect in this component. It is very useful for further treatment.

Family history :

In family history consist of few components. There are;
Name
Age
Sex
Relation of patient
Education 
Occupation
Health status
These are all involve with the family members of patient.

Socio - economic history:

The patient's family, house type, environment of patient's house. Availability of adequate water supply, electricity, shops near the house, hospitals and transport facility. These are all the components in the socio economic history.

Personal history:

Personal hygiene
Diet
Sleep and rest
Elimination
Mobility and exercise

Activities of daily living

Habits( coffee or tea,some daily habits)

Sexual and marital history

Handicapped/deficiency/implants/physically disabled

 These are all the major components of history collection to the patient. It is the basic procedure of nursing care.
